An Experiential Learning Program

The forms below will help guide certificate students through establishing and completing their learning experience. Each should be completed and submitted to in order to complete the program. 

Experience Proposal (pdf)

After establishing contact with your experience site and identifying potential projects, submit your proposal form to make sure you are off to a good start.

Experience Site Agreement (pdf)

Once your experience proposal has been approved, mutually agree to terms and expectations with your site.

Experience Progress Log (pdf)

Maintain your progress log throughout the course of your experience to account for your hours.

Site Satisfaction Form (pdf)

To be completed by your site supervisor at the conclusion of your experience.

Students graduating in May 2019 should complete and submit all certificate requirements by Friday, May 24 at 5pm
